2. Email the completed form to Office@CopperstoneDental.com

2. Complete it at home
3. Bring it into the office with you when you have your appointment

By doing so, your wait time at our practice will be cut exponentially and Dr. Matthew Tuft and Dr. Sean Ziegler will be able to offer their dental services much faster.
